These facilitated workshops will focus on regulatory and survey issues, which
were mandated in 2016 and 2019 for nursing homes and swing bed; trauma
informed care; infection control issues; person-centered care; and using
interdisciplinary teams.

Participants will also have the opportunity to view
Navigators’ presentations from the past year and further discuss best practices
for working with nursing homes, care coordination, continuity of care for patients,
readmission, and how to create, facilitate, and foster partner relationships.

Lunch will be provided, and the cost is $30 per participant.

ABOUT THE SPEAKERS

For over 30 years, Kerry Dunning has specialized in post-acute consultant services, with a specialty in skilled nursing (SNF, SNU, PPS, and CAH swing beds) including Medicare, compliance audits, MDS training, therapy services training, providing mock surveys, and quality measures improvement.

As IRCCO Director of Healthcare Transformation, Trudi Colby provides direct and ongoing education and technical assistance to the network’s participating Accountable Care Organization (ACO) hospitals, clinics, and physician practices. She assists hospital staff and medical providers in improving their quality reporting and individual performance, developing care coordination.
