This two-part webinar series on September 9 and 23 will focus on understanding the internal strengths and challenges as well as how to move from a blaming environment to one of shared accountability.

Topics include:
September 9 - Transitions of Care: Keep Them Safe!
• Define the transitions of care process.
• Learn the relationship between patient safety culture and safe transitions of care.
• Develop a plan to improve your current transition of care process.

September 23 - Just Culture: Accountability for Patient Safety
• Describe the role of human behavior/ choices and system failures that occur when mistakes happen.
• Identify how to conduct an effective investigation following a medical event.

Featured speakers:
Eunice Halverson, MA, CPPS, maintains a passion for improving patient safety. As such, in her role as Patient Safety Specialist for the Center, she enthusiastically assists organizations to improve their patient safety culture and processes.

Kathryn Wire, JD, MBA, CPPS, CPHRM, Center for Patient Safety Executive Director, has extensive experience managing risk and safety functions in acute care, including academic, tertiary, and critical access, physician practices of all sizes, long-term care, and regional health systems, including the initiation of programs within large health systems.
