Position Description: Quality/Case Coordinator, RN

Job Summary: 

Case Coordinator: Monitors and evaluates the presence of and compliance with a clearly-documented plan of care for patients of all ages, which complies with the all regulatory, accrediting and third party payer standards.  Assertively intervenes in a timely manner when variances are identified; facilitates unit-based, multi-disciplinary team meetings to assess, plan and evaluate the level of continuity of care provided to patients.  Facilitates performance improvement activities to promote efficient and effective processes of care and service, which yield optimal clinical, perceptual and financial outcomes.  Advocates for patients through interface with patients’ third party payers so that care and service benefits are being provided according to contractual agreements; provides individual and group educational sessions for patients, physicians, and other members of the multi-disciplinary team related to care and service availability, treatment regimen and regulatory, accrediting, and third party payer guidelines; facilitates patient access to the most clinically appropriate and cost effective level in the continuum of care; performs other related duties.

Quality Coordinator: Facilitate activities to promote efficient and effective processes of care and service, which yield optimal clinical and financial outcomes. Maintain systems to accurately track and trend variances in care and service which can be used for performance improvement purposes. Effectively use clinical and process of care data in monitoring and evaluating outcomes. Facilitate regulatory management activities.

Education and Experience:  Applicants must possess an Associates or Bachelor’s degree in Nursing from an accredited school of nursing; valid and current license with the Illinois Department of Professional Regulation ; at least 5 years clinical experience- preferably in multiple clinical areas.

Knowledge, Skills and Abilities:  Working knowledge of performance improvement methods and tools and facilitation of performance improvement teams preferred; understand content of medical record and possess working knowledge of medical terminology; basic computer skills with Excel, Word, power point preferred; good interpersonal communication skills; critical thinking and problem-solving skills; ability to work independently and unsupervised; ability to understand oral/written instructions; ability to use proper body mechanics when lifting, pushing and pulling, and motor coordination to work with equipment.
