Care management is an important step in healthcare transformation that truly puts the patient at the center - and in control of - his or health own health. Eligible Blue Cross Blue Shield patients will receive a face-to-face visit from the nurse care manager and the practitioner, who will complete a health history. They will also talk to you about your medication, review allergies, and answer any questions you may have concerning your physical and mental health. Your nurse care manager will then develop a personalized action plan, specific to your needs, and work with you to identify goals for improving and managing your health.
This program is proven to reduce the number of patient hospital visits, and those who do go to the hospital have the opportunity to ask questions about their follow up in the privacy of their own homes. This program was developed for qualifying Blue Cross Blue Shield patients with chronic diseases, such as diabetes, congestive heart failure, chronic obstructive pulmonary disease, and hypertension.
